REQUIRED DOCUMENTS

1. Photos showing whole vehicle and extent of damage
2. At least two repair quotations Copy of vehicle logbook
3. Copy of driver’s driving permit
4. Police report for: theft-related cases, where a third party is involved, where repairs may exceed UShs2 million. Statements recorded at Police should also be availed to us.

3rd PARTY REQUIRED DOCUMENTS

Damage to Third Party vehicle:

1. Name of the owner, copy of ID, phone numbers, email, day time physical address
2. Copy of vehicle logbook and a clear photo of the insurance sticker
3. Driver’s name, copy of driving permit, phone contacts
4. Statement by the driver also listing all the damaged parts
5. Full police report including statements recorded at Police
6. At least two repair quotations, demand letter to the client

Damage to Property other than vehicles:

1. Name of the owner and/or designated person such as a manager, copies of IDs, phone numbers, emails, day time physical addresses
2. Full police report including statements recorded at Police
3. At least two repair quotations, demand letter to the client

Bodily Injury:

1. Demand letter to the client, Copy of national ID
2. Clinical and diagnostic notes and receipts from an approved medical practitioner
3. Full police report including statements recorded at Police

Death:

1. Death certificate & Post mortem report
2. Demand letter to the client, Copy of national ID of the deceased
3. Clinical and diagnostic notes and receipts from an approved medical practitioner (if any)
4. Full police report including statements recorded at Police
5. Letter of Introduction of the Beneficiary(ies) or Minutes of the family meeting indicating the beneficiaries & signed by all persons present in the meeting
6. Copies of national IDs of the beneficiaries

CLAIM PROCESS

REQUIRED DOCUMENTS

Kindly return this claim form completed with the following:

1. All clinical and diagnostic notes relating to the injury
2. All medical receipts in the name of the claimant
3. Copies of salary slips for the three months prior to the incident
4. Copy of employee’s company and National ID
5. Police report for road traffic accidents
6. Post mortem report and Death Certificate (for death)

CLAIM PROCESS

1. A client will first notify us in the event of death, accident, or diagnosis of a critical illness.
2. In the event of an incident leading to a claim.
   Notification can be done through email or WhatsApp
3. Once notification is received, the client will be advised on the forms that need to be filled. (You can download the forms below)
4. Once the documents are received, they will be analyzed and communication to the client will be done
5. Once all documents have been analyzed and found to be in order, the claim will be processed, and a discharge voucher will be provided to the claimant.
6. Payment of the claim will then follow

REQUIRED DOCUMENTS

The required documents are dependent on the type of claim.

The forms have all the necessary documents needed clearly indicated on them. It’s also important to note that further documentation can be requested when the need arises.

DOCUMENTS REQUIRED IN CASE OF BODILY INJURY

1. Detailed statement from the insured regarding what happened
2. Demand letter to the insured
3. Full name and copy of national ID or passport of injured party
4. If the injured party is a minor, include full names of both parents and copies of national IDs or passports
5. All clinical and diagnostic notes relating to the injury
6. All medical receipts in the name of the claimant
7. Full police report including statements recorded at Police
